Parental Consent Form (Download Now)
This form must be completed for any minor who receives a spa service or any therapy. This is for clients who are 17 years old or younger. For young minors, a parent must be present in the treatment room. Please inform the spa coordinators of the minor’s age when booking appointments.

This form must be completed when showing expression of Interest for opening of Ganga Spa franchisee centre for a particular location in a city along with your word formatted CV through email for completing the pre-requisite formalities of franchisee’s expression of Interest.
